Behavioral Healthcare, Rooted in Woodland Hills
Elevated Healing was founded in Woodland Hills to close a real gap in San Fernando Valley behavioral healthcare: the space between fragmented care, where mental health and substance use are handled by providers who don’t talk, and generalized care, where both are handled as if they were one condition. Our answer is a specialty-led model — separate primary treatment tracks for mental health and substance use, with structured coordination when both are present.
Our founders, Dr. Kourosh Moradi and Dr. Nicole Fallah, built the program around clinical accountability. Our Medical Director, Warren Taff, MD, brings more than 40 years of psychiatric practice. Our Care Model: Specialty-Led, Distinct Primary Tracks
Elevated Healing operates separate primary treatment tracks for Mental Health and Substance Use, each led by the appropriate clinical specialty from day one. When both are present, our teams coordinate care across tracks so the full clinical picture is addressed without collapsing treatment into a single generalized program.

Who We Serve
The Woodland Hills campus serves adults who need a level of care beyond weekly outpatient therapy or a psychiatric medication check. Common presentations include:
- Mental health primary track — major depression, generalized anxiety, panic disorder, bipolar disorder, PTSD, OCD, ADHD, and related conditions.
- Substance use primary track — opioid, alcohol, stimulant, and cannabis use disorders. Includes evaluation for Medication-Assisted Treatment.
- Coordinated care — when both mental health and substance use are present, care runs through separate primary tracks with a defined coordination process across teams.
We serve adults 18 and older. Access to care does not depend on any single diagnosis: what matters is that a licensed clinician can identify the right primary track and level of care at intake.

The Woodland Hills campus is at 21250 Califa St, Ste 114, Woodland Hills, CA 91367. Access is straightforward:
- From the 101 (Ventura Freeway) — exit at De Soto Avenue or Winnetka Avenue; the campus is a short drive north.
- From the 118 (Ronald Reagan Freeway) — take Winnetka or De Soto south into the West Valley.
- Nearby communities — Warner Center, Winnetka, West Hills, Canoga Park, Tarzana, and Encino are all within a short drive.
- Telehealth — for stable patients and step-down follow-up, we offer video visits available across California.

Structured day program, five days per week, with psychiatric oversight, individual and group therapy, and integrated MAT when clinically indicated.

The first visit is a clinical evaluation, not paperwork. A licensed clinician meets with you, reviews your history, and — together — identifies the primary track and the appropriate level of care. If you’re coming in for substance use care, that evaluation will assess whether Medication-Assisted Treatment is a clinical fit. If you’re coming in for a mental health concern, the psychiatric evaluation looks at diagnosis, prior treatment, and current functioning.
By the end of that visit you should leave with a plan: which track leads, what level of care fits, when your next appointment is, and — when medications are involved — what’s being prescribed and why. Nothing is decided for you without discussion. If a family member or partner needs to be part of the conversation, that can be arranged with your written authorization.
