Day to day, a case manager may coordinate appointments with outside providers, communicate updates to family members (with appropriate consent), assist with insurance verification, identify community resources, and check in with the client to assess how their practical needs are being met. Is case management available at all program levels?
Yes. Case management support is available across our full continuum—Residential (RTC), PHP, IOP, and Outpatient. The intensity and focus adapts as clients step through levels of care, with particular emphasis on transition planning and aftercare coordination.

With the client's consent, case managers serve as a point of communication for family members, providing updates and helping families understand the treatment process. This works in close coordination with our Family Consultations & Education program.
How does case management support discharge and aftercare?
Discharge planning begins well before the end of treatment. Case managers work with the clinical team to develop a comprehensive aftercare plan that may include telehealth follow-up, outpatient therapy, medication management, and community support connections—ensuring continuity of care.
